Paperwork Requirements

Now that you have registered, there are 3 more steps to finalize registration. All 3 of these steps are required and your child will not receive equipment until all 3 steps are complete.

The 3 steps are:

1. Complete Paperwork
2. Upload Paperwork
3. Turn in Paperwork

Details for each step are below.

Step 1: Complete Paperwork

Pop Warner Forms

1. Pop Warner Participation Form - Must be signed by parent and player

2. Physical & Medical History Form 

Pre-populated versions of these forms can be downloaded from the My Account section of our website. See image below

Additional Documents

Birth Certificate  - 1 copy of child's birth certificate (photocopy ok, we dont need the original)
Report Card - 3 copies of the full year 2022-2023 school year report card (1st grade and above)

Step 2: Upload Paperwork

Upload Documents to our website - Once you have all the documents mentioned above please upload digital copies to our website. Below are links to videos that will explain this process. The deadline to complete this is July 1, 2023.

Step 3: Turn in Paperwork

Even though you have uploaded your paperwork, we are still required by Pop Warner to collect hard copies. You can hand in your paperwork at our Fieldhouse on the following dates:

June 12th, July 10th, July 17th, and July 24th
